INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated and experienced Inpatient Case Manager (RN) to join our team at LifeBridge Health. As the Inpatient Care Manager, you will collaborate with the clinical team and medical providers to facilitate discharge planning coordination and intervention, focusing on promoting patient wellness, improving care outcomes, and ensuring efficient health services utilization.

Responsibilities

• Coordinate discharge planning in collaboration with the clinical team and medical providers.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among patients with complex health needs.

Requirements

• Bachelor's degree with working knowledge in a specialized field.
• BSN required from CCNE accredited schools.
• Experience in healthcare settings.
