Job Description

The Housing Authority of Kansas City, Missouri is seeking a qualified Choice Lead Case Manager to direct, manage, and administer the Choice Case Management and Relocation Team while managing their own caseload, providing intensive case management to families formerly living at Chouteau Courts. The incumbent will assess family needs, develop individualized plans for each family member, provide resources and referrals, and track outcomes, in coordination with the United Way of Greater Kansas City.

Responsibilities

  • Supervise daily activities of the Choice Case Management team.
  • Develop individualized care plans including health, education, and employment needs for each family member.
  • Provide assessment of eligible residents to determine service needs.
  • Work with residents on a daily basis to implement individualized care plans using professional judgment and discretion.
  • Assist in obtaining needed services through referrals and related activities.
  • Partner with community agencies to develop resources to assist program participants with quality of life concerns and promote self-sufficiency.
  • Provide crisis interventions and help program participants avert crisis and develop strategies for productive outcomes.
  • Utilize individualized care plans to help families prepare for self-sufficiency and plan relocation services.
  • Assist program participants in finding community resources after relocation, such as employment opportunities and high-performing schools.
  • Mediate relationships between landlords and residents and work with Property Managers to alleviate resident issues.
  • Support residents by guiding case managers in providing resources for employment, education, and health services.

Requirements

  • Bachelor's Degree or Master's degree in social work, psychology, sociology, or other human services related field.
  • Minimum of five (5) years of case management experience. Supervisory experience is preferred.
  • Computer skills and reliable transportation required. Bilingual encouraged. Cultural competence in working with low-income and minority families is desired.

Compensation

Yearly Salary: $57,000
