As a Chronic Care Navigator, you will play a crucial role in supporting patient-centered, team-based care for individuals with chronic conditions. Your responsibilities will focus on utilizing the Phamily platform to manage patient data, identify unmet needs, and enhance communication between patients and their care team.

• Understanding nephrology requirements for optimal population health.
• Collaborating with care teams to establish effective patient care processes.
• Utilizing the Phamily Chronic Care Management platform to support patients with chronic diseases.
• Contributing to quality improvement and care redesign efforts.
• Providing exceptional patient experiences through digital and telephone interactions.
• Assisting in monitoring and correcting patient attribution within the practice.
• Participating in team meetings and upholding compliance with policies and procedures.
