Responsibilities:

• Provide home-based care to patients post-hospital discharge.
• Collaborate with physicians and oversee patient care team.
• Assess patients, document progress, and communicate changes.
• Educate patients and families on care plans and safety.
• Ensure goals are met and facilitate discharge planning.

Requirements:

• Minimum 1 year of acute care experience, home healthcare experience preferred.
• Active RN license in FL and CPR certification.
• Proficiency in Oasis documentation and state/federal regulations.
• Strong clinical skills, communication abilities, and compassion.
