INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. In this full-time role, you will provide discharge planning coordination and intervention in collaboration with the clinical team and medical provider. The ideal candidate will focus on promoting patient wellness, improving care outcomes, optimizing health service utilization, and reducing payment denials for a patient population with complex health needs.

Responsibilities

• Collaborate with clinical team and medical provider for effective discharge planning.
• Promote patient wellness and improve care outcomes.
• Efficiently utilize health services while minimizing payment denials.
• Ensure optimal coordination among multidisciplinary teams.

Requirements

• Bachelor's degree and specialized field knowledge.
• BSN required from CCNE accredited schools.
• Experience in utilization review or case management preferred.
