LPN Quality Care Coordinator

Position Summary:
The LPN Quality Care Coordinator at Phamily collaborates with providers to manage care plans for chronically ill patients. This role focuses on patient education, follow-up on ER visits, and triaging medical concerns over the phone.
Responsibilities:
- Manage care for patients with chronic conditions.
- Provide patient education and support treatment plans.
- Follow up on recent ER visits and oversee Transition Care Management services.
- Perform general nursing duties and medical procedures.
- Conduct phone triage for patients with medical concerns.
Requirements:
- Completion of an accredited LPN Program.
- Minimum 2 years of LPN experience, 1+ year with IV Therapy and catheterization.
- Current Washington State LPN License and ACLS or BLS certification.

Work Environment:
