Job Description

We are currently looking for a motivated Licensed Practical Nurse (LPN) Quality Care Coordinator to join our team at Phamily. In this role, you will work closely with our medical providers to manage care plans for chronically ill patients. Your responsibilities will include providing patient education, following up on emergency room visits, and triaging patients' medical concerns. Additionally, you will perform general nursing duties and medical procedures within your scope of practice.

Responsibilities

  • Manage care for patients with chronic conditions in collaboration with medical providers.
  • Provide patient education and support to ensure adherence to treatment plans.
  • Follow up with patients after emergency department visits.
  • Coordinate Transition Care Management services for hospitalized patients.
  • Perform medical procedures and general nursing duties including phone triage.

Requirements

  • Completion of an accredited LPN Program.
  • Minimum 2 years of experience as an LPN.
  • Experience with IV Therapy and catheterization preferred.
  • Current Washington State LPN License required.
  • Current ACLS or BLS certification preferred.

Work Environment

This role will primarily work in an office setting, interacting with patients over the phone and in person. Standard office equipment will be used with fluent English communication skills required.

Physical Demands

The position involves both sedentary and medical care duties, including contact with bodily fluids. Occasional lifting up to 20-30 pounds may be required.

Education and Experience

  • Completion of an accredited LPN Program.
  • 2+ years of LPN experience required.
  • Experience with IV Therapy and catheterization preferred.
  • Current Washington State LPN License required.
  • Current ACLS or BLS certification preferred.
