Job Description

We are looking for an experienced RN Care Coordinator to join our Ambulatory Care team at Denver Health. As an RN Care Coordinator, you will play a crucial role in coordinating care for assigned patients, ensuring quality outcomes through collaboration and assessment. This position involves working closely with patients and their families, healthcare teams, and community resources to deliver comprehensive care while adhering to regulatory requirements.

Requirements

• Associate's Degree in Nursing and RN license.
• Three years of clinical experience.
• BLS certification.
• Strong communication and problem-solving skills.
• Proficiency in Microsoft Outlook, Word, and Excel.
