Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a crucial member of our healthcare institution, you will provide invaluable clinical social work services and psychosocial assessments to patients, their families, and caregivers. Your responsibilities will encompass addressing a wide range of issues, including adjustment challenges, financial concerns, family dynamics, end-of-life counseling, and discharge planning.
Responsibilities
- Offer clinical social work services and psychosocial assessments to patients and their families.
- Address various challenges such as adjustment to institutional living, financial worries, and family relationships.
- Provide counseling on acceptance of medical diagnoses, aging issues, powerlessness, and end-of-life discussions.
- Contribute to discharge planning and support patients and families in difficult times.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree is preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) is preferred.
- Licensed Graduate Social Worker (LGSW) certification.
