Job Description
We are looking for a dedicated Social Worker to join our Care Management team at Carroll Hospital, part of the LifeBridge Health system. This part-time position with a weekend commitment offers an opportunity to make a real impact on patient care. As a Social Worker, you will collaborate with healthcare professionals to ensure efficient care delivery and coordinate with community agencies to address various social issues affecting patients. This role requires a master's degree in social work, 2-4 years of relevant experience, and preferably an LCSW-C license.
Responsibilities
- Partner with healthcare team members to facilitate efficient care for inpatients.
- Coordinate with community agencies to address social issues such as abuse, neglect, substance abuse, and mental health.
- Link patients to financial resources and support services.
- Work closely with Emergency Department staff as needed.
Requirements
- Master's degree in social work.
- 2-4 years of recent social work experience, preferably in a medical setting.
- Preference for LCSW-C license, consideration for LMSW license.
