Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and conduct psychosocial assessments for patients, addressing a variety of challenges they may face.

Responsibilities

Offer support and counseling to patients/residents and their families on various issues such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life matters.
Assist individuals in coping with medical diagnoses, treatments, aging-related changes, and discharge planning.

Requirements

Bachelor's degree in Social Work or related field.
1-3 years of relevant experience.
Licensed Bachelor Social Worker (LBSW), Licensed Master Social Worker (LMSW), or Licensed Graduate Professional Counselor (LGPC) credentials.
