Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, focusing on various common issues such as adjustment to institutional living, financial concerns, and family relationships. You will play a crucial role in assisting patients and their families through challenging situations related to medical diagnoses, aging, discharge planning, and end-of-life matters.

Responsibilities

• Provide clinical social work services to patients and families.
• Offer psychosocial assessments and support for various challenges.
• Assist patients in coping with medical diagnoses and procedures.
• Participate in discharge planning and bereavement counseling.

Requirements

• Bachelor's degree with relevant professional knowledge.
• 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
