Job Description
We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various common challenges such as adjustment issues, financial concerns, family dynamics, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and support transitions for patients.
- Collaborate with interdisciplinary teams to address patient needs.
Requirements
- Bachelor's degree and 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Ability to work with diverse populations and handle sensitive situations with empathy.
