SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families facing various challenges related to institutional living, financial concerns, family dynamics, medical diagnoses, aging, discharge planning, and end-of-life issues.

Responsibilities

• Offer counseling and support to patients and families in times of need.
• Conduct psychosocial assessments to address emotional and social needs.
• Collaborate with multidisciplinary teams for comprehensive patient care.
• Assist with discharge planning and transitions of care.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong knowledge and understanding of social work theory and practice.
• Compassionate and empathetic approach to patient care.
