SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common problems related to institutional living, financial concerns, family relationships, medical procedures, aging, discharge planning, and end-of-life issues.

Responsibilities

• Conduct psychosocial assessments and provide counseling services.
• Assist patients and families in adjusting to medical diagnoses and treatments.
• Develop discharge plans and provide support related to death and dying.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
