SOCIAL WORKER PRN

Job Description

We are currently seeking a compassionate Social Worker for PRN (As Needed) position at LifeBridge Health in Baltimore, MD, specifically at Levindale Hospital. The successful candidate will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a variety of common issues. This role involves working with patients on adjustment to institutional living, financial concerns, family relationships, acceptance of medical procedures, discharge planning, and counseling related to aging, the end of life, and more.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Assist patients with various challenges including adjustment, financial concerns, family relationships, and end-of-life counseling.
• Collaborate with healthcare teams to ensure comprehensive care for patients and residents.

Requirements

• Bachelor's degree or equivalent in social work or related field.
• 1-3 years of experience as a Social Worker.
• Licensed Social Worker (LBSW/LMSW/LGPC).
