SOCIAL WORKER PRN

Job Description

LifeBridge Health is currently seeking a PRN Social Worker to join our team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, focusing on various common problems such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Offer psychosocial support and counseling to patients and their families.
• Assess patient needs and develop individualized care plans.
• Collaborate with healthcare professionals to ensure comprehensive care.
• Conduct discharge planning and provide support for end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
