Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. As a clinical social worker, you will be responsible for providing psychosocial assessments and support to patients, residents, and their families. Your role will involve addressing a wide range of common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and address common problems related to medical conditions and aging.
• Assist in discharge planning and provide counseling support.
• Collaborate with healthcare teams to ensure comprehensive care.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in a healthcare setting.
• Strong knowledge of social work theory and practice.
• Compassionate, empathetic, and able to work in challenging situations.
