SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address issues related to adjustment to healthcare settings and aging.
• Collaborate with healthcare teams for discharge planning.
• Offer support and guidance regarding financial and family matters.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
• Knowledge of social work theory and practice.
