Job Description

Responsibilities
- Evaluate and treat patients, establishing outcomes-centered plans.
- Deliver effective care management across a patient's episode, driving consistent positive outcomes.
- Operate at the top of your license, appropriately delegating care amongst the clinic care team.
Qualifications
- Must be licensed or license eligible in the state of application.
