We are looking for a dedicated Registered Nurse to join our Intake and Hospital Liaison team at UVM Health Network Home Health & Hospice. This role is crucial for seamless transitions in patient care, working closely with providers, clinicians, and patients/families to facilitate the referral process. As a Registered Nurse in this position, you will play a key role in educating patients and providers, coordinating discharges, reviewing clinical documentation, and ensuring patient eligibility for services.

Requirements

• Associate's degree in nursing.

• Minimum of two years of RN experience.
