Job Summary:

The CSC Case Manager at PermiaCare will be responsible for providing psychoeducation, psychosocial rehabilitation, and case management services to individuals enrolled in the Coordinated Specialty Care-First Episode Psychosis program. This position involves collaborating with a multidisciplinary team to support individuals between 15 to 30 years old in their recovery journey.

Key Responsibilities:

• Provide direct services and support in line with recovery plans.
• Work with individuals and families to achieve recovery goals.
• Ensure required service delivery targets are met consistently.
• Offer education and outreach on First Episode Psychosis.
• Facilitate integration with essential services and resources.

Qualifications:

• Bachelor's degree in psychology, social work, or related field.
• 1+ year of experience in social services preferred.
• Valid Texas Driver's license and QMHP certification required.
