HOME CARE REGISTERED NURSE

Job Description

We are currently looking for a dedicated HOME CARE REGISTERED NURSE (RN) to join our compassionate team at Liberty Healthcare Management. As a Full-Time RN, you will play a vital role in providing professional nursing services to our patients, promoting favorable outcomes through collaborative care, and ensuring accurate documentation for quality improvement.

Responsibilities

• Participate actively in interdisciplinary care planning.
• Provide nursing services following established care plans.
• Promote positive outcomes through collaborative practices.
• Accurately document provided services for coordination and compliance.
• Develop positive interpersonal relationships with patients and colleagues.
• Take personal accountability for professional growth and task accomplishment.

Requirements

• Completion of nursing education from an approved school.
• Valid RN licensure in the state and at least one year of clinical nursing experience.
• Home health experience preferred.
• Proficiency in medical-surgical nursing principles.
• Excellent clinical nursing and communication skills.
• Computer literacy and problem-solving abilities.
• CPR certification required.
