INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. In this role, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions, aiming to enhance patient wellness, improve care outcomes, utilize health services efficiently, and reduce payment denials for patients with complex health needs.

Responsibilities

• Provide discharge planning coordination and intervention for inpatient cases.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize payment denials for patients with complex health needs.

Requirements

• Equivalent to a Bachelor's degree with working knowledge in a specialized field.
• BSN required from CCNE accredited schools.
• Experience in utilization review, clinical care coordination, patient care integration, or RN case management would be advantageous.
