Licensed Practical Nurse

Job Description

We are looking for a skilled and compassionate Licensed Practical Nurse to join CNY Family Care, LLP. In this role, you will be responsible for providing direct patient care, implementing preventive measures, and ensuring the wellbeing of our patients. As a part of our patient-centered medical home, you will have the opportunity to work in a supportive and inclusive environment that fosters excellence and teamwork.

Responsibilities

• Obtaining vitals, intake processes, and administering injections.
• Working alongside providers to assist with patient visits and performing ECGs.
• Engaging in telephone triage, placing orders, refilling prescriptions, and conducting phlebotomy.

Job Requirements

• Current valid New York State licensure as a Licensed Practical Nurse.
• Strong critical thinking skills and excellent customer service abilities.
• Prior experience in a busy medical office is preferred but not required.
