Job Description

We are looking for a passionate PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, and discharge planning. The ideal candidate will have a Bachelor's degree in social work and 1-3 years of experience, with licensure as LBSW, LMSW, or LGPC.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address common social work issues such as financial concerns and discharge planning.
• Collaborate with healthcare professionals to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of social work experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong understanding of social work theory and practice.
