Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Offer clinical social work services to patients/residents and their families.
• Assess and address common challenges related to institutional living, financial issues, family dynamics, and medical processes.
• Provide counseling on discharge planning, death, and dying.

Requirements

• Bachelor's degree or equivalent in social work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
