Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services, conduct psychosocial assessments, and offer support to patients, residents, and their families in various areas such as institutional living adjustment, financial concerns, and counseling related to aging and end-of-life issues.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Support patients and families with adjustment to institutional living, financial concerns, and end-of-life counseling.
• Assist with discharge planning and related counseling.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
