SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker at Levindale Hospital, you will provide clinical social work services to patients and their families, addressing various social and emotional challenges they may face during their stay.

Responsibilities

• Conduct psychosocial assessments for patients/residents and families.
• Offer counseling and support for adjustment to institutional living, financial concerns, family relationships, and other related issues.
• Assist with discharge planning and provide counseling for end-of-life matters.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
