Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Offer clinical social work services and psychosocial assessments.
• Address common problems such as adjustment to institutional living and family relationships.
• Provide counseling related to death and dying.
• Collaborate with healthcare teams to enhance patient care.

Requirements

• Basic professional knowledge and Bachelor's degree.
• 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
