Job Description

As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment issues, financial concerns, and discharge planning. Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments to address common challenges.
• Offer counseling and support related to adjustment to institutional living, aging, and end-of-life care.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience in social work.
• LBSW, LMSW, or LGPC certification.
• Strong interpersonal and communication skills.
