SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common problems related to institutional living, financial concerns, family relationships, aging, and more.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common issues such as adjustment to institutional living, financial concerns, and discharge planning.
• Offer counseling related to death and dying, powerlessness, and family relationships.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of theory and practice in social work.
