Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges such as adjustment to institutional living, financial concerns, and discharge planning. Join us in our mission to CARE BRAVELY for over 1 million patients annually in Maryland.

Responsibilities

• Deliver clinical social work services and psychosocial assessments.
• Address challenges related to adjustment, family relationships, and aging.
• Provide counseling on topics like death and dying.
• Contribute to discharge planning efforts.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
