Job Description
We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face during their stay.
Responsibilities
- Offer support and counseling to patients and families facing emotional difficulties.
- Assess and address financial concerns and assist with discharge planning.
- Provide guidance on family relationships, aging-related changes, and end-of-life care.
- Collaborate with healthcare teams to ensure comprehensive patient care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience as a Social Worker.
- Licensed Social Worker (LBSW, LMSW, LGPC).
- Strong knowledge of social work theory and practice.
