SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges and needs.

Responsibilities

• Provide psychosocial assessments to patients and families.
• Address issues such as adjustment to institutional living, family relationships, financial concerns, discharge planning, and counseling related to death and dying.
• Collaborate with healthcare teams to support patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in clinical social work.
• LBSW/LMSW/LGPC certification.
• Strong understanding of social work theory and practice.
