SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face. This includes assisting with adjustment to institutional living, financial concerns, family relationships, medical procedures, aging-related changes, discharge planning, and end-of-life counseling.

Requirements

• Bachelor's degree with 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
• Knowledge of theory and practice in social work.
• Strong communication and interpersonal skills.
