SOCIAL WORKER PRN

Job Description

We are looking for a passionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a valued member of our Allied Health team at Levindale Hospital, you will provide expert clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges with empathy and professionalism.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address issues related to institutional living, financial concerns, family dynamics, medical understanding, aging, discharge planning, and end-of-life support.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification.
