Job Description
We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face during their healthcare journey.
Responsibilities
- Offer support and counseling to patients/residents and families on a range of issues such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.
- Conduct psychosocial assessments to identify needs and develop appropriate care plans.
- Collaborate with healthcare professionals to ensure holistic care for patients.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- LBSW/LMSW/LGPC certification.
- Strong communication and interpersonal skills.
