Job Description
We are looking for a PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, addressing a variety of common issues including adjustment to institutional living, financial concerns, family relationships, and discharge planning. You will be part of a dynamic healthcare environment where you can make a difference in people's lives.
Responsibilities
- Conduct psychosocial assessments of patients and families.
- Provide counseling related to various emotional and social challenges.
- Collaborate with healthcare professionals to develop and implement care plans.
- Participate in discharge planning and coordination of post-hospital care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of experience in social work.
- Licensed as an LBSW, LMSW, or LGPC.
- Understanding of medical procedures and patient care.
