SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, assisting them and their families with various challenges they may encounter.

Responsibilities

• Conduct psychosocial assessments to identify patient needs.
• Assist in addressing issues such as adjustment to institutional living, financial concerns, and family relationships.
• Provide counseling on medical procedures, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
