SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker to join our team on a PRN basis at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and conduct psychosocial assessments for patients, families, and residents facing various challenges.

Responsibilities

• Offer support and counseling to individuals and families dealing with institutional living, financial stress, family dynamics, medical issues, aging, and end-of-life planning.
• Collaborate with healthcare professionals to develop effective care plans and discharge arrangements.
• Provide emotional support and guidance to individuals coping with difficult life situations.
• Document patient interactions following hospital policies and regulations.

Requirements

• Bachelor's degree in Social Work or related field.
• Current licensure as LBSW/LMSW/LGPC.
• 1-3 years of relevant experience in healthcare settings.
• Strong interpersonal and communication skills.
