Job Description

We are looking for a passionate Social Worker (PRN) to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common issues. This includes adjustment to institutional living, financial concerns, family relationships, counseling related to medical procedures, discharge planning, and support related to end-of-life care.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common issues such as adjustment to institutional living and family relationships.
• Offer counseling related to medical procedures and end-of-life care.
• Assist in discharge planning.

Requirements

• Bachelor's degree or equivalent with working knowledge in a specialized field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
