Job Description

We are looking for a passionate and dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients and families facing various challenges and transitions. In this role, you will address issues such as adjustment to institutional living, financial concerns, family relationships, medical treatment understanding, aging-related changes, discharge planning, and end-of-life counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common problems related to institutional living, financial concerns, family relationships, medical treatment understanding, aging-related changes, and end-of-life counseling.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
