Job Description

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Conduct psychosocial assessments and address common adjustment challenges.
• Offer counseling and support related to aging, discharge planning, and end-of-life issues.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
