Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position is on a PRN basis at Levindale Hospital, offering flexibility in your work schedule. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling on end-of-life issues.
Responsibilities
- Conduct psychosocial assessments of patients and families.
- Provide counseling and support related to various emotional and social challenges.
- Assist with discharge planning and end-of-life care.
- Collaborate with healthcare professionals to ensure comprehensive patient care.
Requirements
- Bachelor's degree or equivalent professional knowledge.
- 1-3 years of experience in social work.
- LBSW/LMSW/LGPC certification.
- Strong interpersonal and communication skills.
