SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face. This includes assisting with adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling for end-of-life care.

Responsibilities

• Deliver clinical social work services and psychosocial assessments.
• Address common patient and family problems related to health, relationships, aging, and end-of-life care.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
