SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join the team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of challenges related to medical conditions, family dynamics, aging, and end-of-life care.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and support patients in transitioning back to their communities.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of related experience.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
