SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker (PRN) to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker at Levindale Hospital, you will be providing clinical social work services and psychosocial assessments to patients, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Provide psychosocial assessments to patients and families.
• Offer counseling on adjustment issues and end-of-life planning.
• Collaborate with healthcare teams for discharge planning.
• Support patients in coping with medical diagnoses, treatments, and aging-related changes.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensure as LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
