Job Description

We are currently looking for a skilled Social Worker (PRN) to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As a member of our Allied Health team, you will provide vital clinical social work services and psychosocial assessments to patients, residents, and their families.

Responsibilities

  • Offer psychosocial support and counseling to address various challenges faced by patients and families.
  • Assist in discharge planning and end-of-life counseling.
  • Provide guidance on financial concerns, family relationships, and institutional living adjustments.
  • Collaborate with medical teams to ensure holistic patient care.

Requirements

  • Bachelor's degree and 1-3 years of relevant experience.
  • Licensed as LBSW, LMSW, or LGPC.
  • Strong understanding of social work theory and practice.
