SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services to patients/residents and families, addressing various challenges related to institutional living, relationships, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling and support to individuals and families.
• Assist in discharge planning and end-of-life care.
• Address financial concerns and family relationships.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
